Our professional in-home nurses specialize in various medical services that address our clients’ needs. We offer these programs to individuals eligible for private home care. Furthermore, we have trained our nurses to establish positive relationships with our clients to effectively deliver healthcare interventions to their homes. Our nurses will report our clients’ health changes to the supervisor regularly.
The responsibilities of our nurses include but not limited to:
- Regularly assess the nursing needs of the client
- Participate in the establishment and implementation of the client’s care plan
- Provide nursing services as needed and in accordance with the client’s care plan
- Report problems and progress of the client to supervisory personnel or the client’s personal physician
Nursing services are performed by licensed practical nurse (LPN) and registered nurse (RN) but not limited to:
- Wound care
- Intravenous/injection administration Catheter management
- Disease education and management such as diabetes and CHF
- Tube feedings and management
- Specimen collection such as blood and urine
- Cardiopulmonary Care such as breathing treatments and suctioning
- Patient and family centered health education
- Vital signs and glucose monitoring
- Collaboration with patient’s primary care providers for patient’s centered care
